A new class is admitted to the BSDH/MDT program once each year in the fall.
Using this same application below you can choose to apply to the BSDH/MDT program only, the Dental Hygiene program only or to both programs. Click here for more information about Dental Hygiene.

Applicants who have never attended the University of Minnesota must submit a transfer application.
Applicants who are currently attending, or have previously attended the University of Minnesota must submit a change of college form.

Step 3
Submit Three Letters of Recommendation.
- Two letters from college-level science or math professors from whom you have taken a course
- One letter from an employer or a supervisor from a volunteer position.
Letters of recommendation must be submitted to us directly from the recommenders themselves, and may be submitted in the following ways:
