- CV
- Personal Statement
- Three (3) Professional Evaluations
- TOEFL score of 102 or higher (if applicant is not from the US or Canada)

We encourage you to submit your application in early July. The deadline to submit applications and all required supplemental materials is August 1.
Due to the pandemic, we have decided not to accept externs during the summer semester. If you are interested in doing a 3-5 day externship in September or October, please reach out to the program Administrative Specialist, Shelby LaFreniere, at [email protected] with your CV and a brief letter of interest in August. We are looking for highly qualified individuals (top 1/3 of their dental school class) with a great interest in periodontics and implantology.
International applicants whose primary language is not English, and who have not obtained a degree in the U.S., are required to submit a TOEFL (Test of English as a Foreign Language) score taken within two years of their application deadline. Applicants must achieve a minimum > or = 102 IBT score to be considered by the admissions committee.
All applicants are required to pay the Division of Periodontology a supplemental application processing fee. This fee is $100 for domestic applicants and $125 for international applicants. Please pay the fee on our online portal. Applications will not be considered complete until the supplemental fee is received.
If you have any additional questions, please contact the program Administrative Specialist, Shelby LaFreniere, at [email protected].
